=== Clinical trials === In addition to the increased production at the Dunn School, commercial production from a pilot plant established by Imperial Chemical Industries became available in January 1942, and Kembel, Bishop and Company delivered its first batch of 200 imperial gallons (910 L) on 11 September. Florey conducted a second series of clinical trials. Ethel was placed in charge, but while Florey was a consulting pathologist at Oxford hospitals and therefore entitled to use their wards and services, Ethel, to his annoyance, was accredited merely as his assistant. Doctors tended to refer patients to the trial who were in desperate circumstances rather than those who were the most suitable candidates for treatment, but when penicillin did succeed anyway, confidence in its efficacy rose. Harry Lambert, a friend of Fleming's dying from a meningococcal infection, became the twelfth case on 5 August 1942. He survived, but someone at St. Mary's Hospital leaked the result to the press, resulting in an editorial in The Times on 27 August. Florey was appalled; this could only create a public demand for penicillin when all available supplies were needed for the clinical trials. On 25 September, Florey met with Sir Cecil McAlpine Weir, the Director-General of Equipment and Stores at the Ministry of Supply, who promised overriding priority for the mass production of penicillin. == Side effects and contraindications == The withdrawal of troglitazone has led to concerns of the other thiazolidinediones also increasing the incidence of hepatitis and potential liver failure, an approximately 1 in 20,000 individual occurrence with troglitazone. Because of this, the FDA recommends two to three month checks of liver enzymes for the first year of thiazolidinedione therapy to check for this rare but potentially catastrophic complication. To date, 2008, the newer thiazolidinediones, rosiglitazone and pioglitazone have been free of this problem. The main side effect of all thiazolidinediones is water retention, leading to edema, generally a problem in less than 5% of individuals, but a big problem for some and potentially, with significant water retention, leading to a decompensation of potentially previously unrecognized heart failure. Therefore, thiazolidinediones should be prescribed with both caution and patient warnings about the potential for water retention/weight gain, especially in patients with decreased ventricular function (NYHA grade III or IV heart failure). Though older studies suggested there may be an increased risk of coronary heart disease and heart attacks with rosiglitazone, pioglitazone treatment, in contrast, has shown significant protection from both micro- and macro-vascular cardiovascular events and plaque progression. These studies led to a period of Food and Drug Administration advisories (2007 – 2013) that, aided by extensive media coverage, led to a substantial decrease in rosiglitazone use.

==== Ovarian function ==== In 2021, a phase I non-randomized clinical trial, the first in humans, evaluated the safety and potential benefits of injecting autologous adipose-derived stem cells (ADSCs) into the ovaries of women with premature ovarian failure (POF). Nine patients received varying doses of ADSCs (5, 10, or 15 million cells), and were followed for up to 12 months. The results showed that there were no serious side effects reported, menstruation resumed in 4 out of 9 patients, serum FSH levels decreased in 4 patients, indicating some hormonal improvement. Additionally, ovarian volume, AMH, and antral follicle count (AFC) showed variable results with no significant differences between groups. The study concluded that intra-ovarian ADSC therapy appears safe and feasible, with some signs of improved ovarian function, though results were inconsistent. Larger randomized trials are needed for confirmation.
