=== Skeletal system === Most of the readily visible signs are associated with the skeletal system. Many people with Marfan syndrome grow to above-average height, and some have disproportionately long, slender limbs with thin, weak wrists and long fingers and toes. The Steinberg sign, also known as the thumb sign, is one of the clinical examination tests for Marfan disease in the hands. It is a clinical test in which the tip of the thumb extends beyond the palm when the thumb is clasped in the clenched hand. Besides affecting height and limb proportions, people with Marfan syndrome may have abnormal lateral curvature of the spine scoliosis, thoracic lordosis, abnormal indentation (pectus excavatum) or protrusion (pectus carinatum) of the sternum, abnormal joint flexibility, a high-arched palate with crowded teeth and an overbite, flat feet, hammer toes, stooped shoulders, and unexplained stretch marks on the skin. It can also cause pain in the joints, bones, and muscles. Some people with Marfan have speech disorders resulting from symptomatic high palates and small jaws. Early osteoarthritis may occur. Other signs include limited range of motion in the hips due to the femoral head protruding into abnormally deep hip sockets.

=== Other symptoms === Breathing difficulties can occur, resulting from neuromyotonic activity of the laryngeal muscles. Laryngeal spasm possibly resulting from neuromyotonia has been described previously, and this highlights that, in patients with unexplained laryngospasm, neuromytonia should be added to the list of differential diagnoses. Studies have shown subtly decreased metabolism on positron emission tomography (PET) and single photon emission computed tomography (SPECT) in the left inferior frontal and left temporal lobes. and or basal ganglia hypermetabolism. Ancillary laboratory tests including MRI and brain biopsy have confirmed temporal lobe involvement. Cranial MRI shows increased signal in the hippocampus. Cerebral spinal fluid (CSF) shows normal protein, glucose, white blood cell, and immunoglobulin G (IgG) levels, but there are weak oligoclonal bands, which are absent in the blood serum. Marked changes in circadian serum levels of neurohormones and increased levels of peripheral neurotransmitters were also observed. The absence of morphological alterations of the brain pathology, the suggestion of diffusion of IgG into the thalamus and striatum, more marked than in the cortex (consistent with effects on the thalamolimbic system) the oligoclonal bands in the CSF and the amelioration after PE all strongly support an antibody-mediated basis for the condition. The Bradford assay, a colorimetric protein assay, is based on an absorbance shift of the dye Coomassie brilliant blue G-250. The Coomassie brilliant blue G-250 dye exists in three forms: anionic (blue), neutral (green), and cationic (red). Under acidic conditions, the dye is red; when it is deprotonated, the red form of the dye is converted into its blue form, which can bind to the protein being assayed. If there is no protein present, then the solution will remain brown. Dye-protein interactions are driven by electrostatic interactions between positively charged arginine (and to a lesser extent, lysine and histidine) and the deprotonated negatively charged sulfonate groups on the dye, as well as hydrophobic interactions with the aromatic amino acids (tryptophan, tyrosine, phenylalanine). Binding of the dye to the protein stabilizes the anionic form of the dye, which can be detected colorimetrically by a shift from 465 nm to 595 nm. The cationic (unbound) form is red and has an absorption spectrum maximum at 465 nm, whereas the anionic bound form has an absorption spectrum maximum at 595 nm. The increase of absorbance at 595 nm is proportional to the amount of bound dye, and thus to the amount (concentration) of protein present in the sample. Unlike other protein assays, the Bradford protein assay is less susceptible to interference by various chemical compounds such as sodium, potassium or even carbohydrates like sucrose, that may be present in protein samples. An exception of note is elevated concentrations of detergent.
Von Willebrand factor is normally synthesized in the endoplasmic reticulum of endothelial cells lining blood vessels (and also in megakaryocytes), and it is then packaged into multimers (many strands of vWF connected by disulfide bonds) by the Golgi and stored in Weibel-Palade bodies as a helical spiral of multiple multimers. When vWF is secreted by endothelial cells, the multimers are cleaved by the enzyme ADAMTS13 and vWF circulates in the plasma in a coiled and inactive form. When there is damage to a blood vessel (due to trauma or other factors) collagen under the blood vessel lining is exposed. When vWF comes into contact with exposed collagen it uncoils and binds to the collagen. Circulating platelets bind to vWF using their GpIb-alpha surface protein which binds to a specific area on the uncoiled vWF strand (The A1 domain binding site). Upon binding, the platelets become activated and irregularly shaped which attracts more platelets to the area of vascular damage to form a platelet plug in the blood vessel wall and stop the bleeding. In VWD, vWF is either deficient (type 1 disease), dysfunctional (type 2 disease), or is completely absent (the severe type 3 disease) leading to dysfunction in the above mechanism to stop bleeding. Circulating vWF also binds to coagulation factor VIII preventing it from being degraded. Factor VIII is involved in the coagulation cascade to also prevent excessive bleeding. == Parent isotope (68Ge) source == The parent isotope germanium-68 is the longest-lived of the radioisotopes of germanium. It has been produced by several methods. In the U.S., it is primarily produced in proton accelerators: the reaction is 69Ga(p,2n)68Ge (the target is the more abundant constituent of natural gallium). At Los Alamos National Laboratory, it may be separated out after proton irradiation of Nb-encapsulated gallium metal. It is also produced at Brookhaven National Laboratories by 40 MeV proton irradiation of a gallium metal target. A Russian source produces germanium-68 from accelerator-produced helium ion (alpha) irradiation of zinc-66, again after knockout of two neutrons, in the nuclear reaction 66Zn(α,2n)68Ge.
=== Fluorinated cyclooctynes === The cyclooctane derivative OCT was the first one developed for Cu-free click chemistry; it had only ring strain to drive the reaction forward, and the kinetics were barely improved over the Staudinger ligation. After OCT and MOFO (monofluorinated cyclooctyne), the difluorinated cyclooctyne (DIFO) was developed. An improved synthetic approach to a monofluorosubstituted cyclooctyne (MFCO) was introduced that could easily be converted to a useful reactive intermediate for bioconjugation applications, although the reactivity was somewhat slower than DIFO. The MFCO demonstrated excellent stability characteristics for long-term storage. The substituted cyclooctyne is activated for a 1,3-dipolar cycloaddition by its ring strain and electron-withdrawing fluorine substituents, which allows the reaction to take place with kinetics comparable to the Cu-catalyzed Huisgen cycloaddition. Ring strain (~18 kcal/mol) arises from the deviation of the bond angles from the ideal 180° to form an eight-membered ring, the smallest of all cycloalkynes. The electron-withdrawing fluorine substituents were chosen due to their synthetic ease and compatibility with living biological systems. Additionally, the group cannot produce cross-reacting Michael acceptors that could act as alkylating agents toward nucleophilic species within cells. Like most cyclooctynes, DIFO prefers the chair conformation in both the ground state and the minimum energy traction path, although boat transition states may also be involved.
