In this process, each of the peptides in the cycle are connected to one another one at a time by coupling reactions. In this case, first a valine residue is protected by the Fmoc protecting group to protect the amine group in the amino acid. The peptide is then coupled to an OICh (cholestene) group. OICh is a protecting group used to protect the carboxyl group in the amino acid. Next, the Fmoc group is removed to make an amine by using diethyl amine (Et2N). The next amino acid in the peptide chain is then added to the amine in the same type of coupling reaction, and the process is repeated until the chain form of the cycle is complete. The OICh group is removed by using powdered zinc with acetic acid.

The L-cells are open-type triangular epithelial cells directly in contact with the lumen and neuro-vascular tissue and are accordingly stimulated by various nutrient, neural and endocrine factors. GLP-1 is released in a biphasic pattern with an early phase after 10–15 minutes followed by a longer second phase after 30–60 minutes upon meal ingestion. As the majority of L-cells are located in the distal ileum and colon, the early phase is likely explained by neural signalling, gut peptides or neurotransmitters. Other evidence suggests the L-cells located in the proximal jejunum are sufficient to account for the early-phase secretion through direct contact with luminal nutrients. Less controversially, the second phase is likely caused by direct stimulation of L-cells by digested nutrients. The rate of gastric emptying is therefore an important aspect to consider, as it regulates the entry of nutrients into the small intestine where the direct stimulation occurs. BMS and Pfizer worked quickly to block generics from being created, and in August 2020, they won a patent infringement lawsuit against Sigmapharm, Sunshine Lake, and Unichem, after previously settling patent cases against 25 other companies. In September 2021, a Federal Circuit Court upheld the ruling. The result is that apixaban generics will most likely not be available in the United States until at least 2026, but possibly 2031. In July 2022, the Canadian generic drug company, Apotex Inc., obtained approval for marketing of apixaban. Pfizer reported revenue of US$6.747 billion for Eliquis in 2023. Apixaban is one of ten medications covered by price negotiations in the US under the Inflation Reduction Act. The negotiations, conducted by the Centers for Medicare & Medicaid Services, apply to pricing for Medicare recipients. The results of the negotiations were announced in August 2024, and Medicare's negotiated price for a 30-day supply of Eliquis is $231, a 56% decrease from the 2023 list price of $521.
